What the form collects
Name, email, general area of Minnesota, interest in the first gathering, areas of interest, whether you would like to help organize, and your acknowledgment of this notice. Caribbean family origin, plans to bring family or children, and a short message are optional.
What we do not ask for
We do not request government ID, DNA tests or percentages, birth certificates, dates of birth, Social Security numbers, genealogy documents, detailed family trees, or immigration information.
How the information is used
Information is used to respond to your interest and coordinate relevant community activities. It is not an application for membership or enrollment. We do not sell it or use it for advertising.
Current submission status
A form marked as a demonstration does not send or save the information entered. Production submissions remain unavailable until a provider is configured. A confirmation appears only after a configured provider accepts the submission.
Access, retention & requests
For privacy questions or requests for access, correction, or deletion, contact kiki.stevens@tainominnesota.org. Formspree processes and stores submissions and delivers copies to the organizer’s inbox. Access is limited to the organizer. The organizer will delete interest submissions from both Formspree and the inbox within six months of receipt, or sooner on request.
Tracking & technical services
This application adds no analytics, advertising trackers, or marketing cookies. Hosting and submission providers may process technical request information; their specific practices must be disclosed when the services are selected. Form anti-spam checks use a hidden field and the time taken to complete the form.
Children & sensitive information
Adults should complete the form. Please do not provide children’s names, ages, identity documents, or sensitive family details. General family attendance information is optional.
Future systems
Genealogy, identity, enrollment, and membership records would require a separate security and privacy architecture and community-approved governance.
